110學年度住宿生應於入住前起至服務台簽署自我健康聲明書 Notice for students moving into the dorms in 2021

因應國內新冠疫情，並配合中央流行疫情指揮中心訂定之境外生入境作業，以及教育部公布之「大專校院住宿防疫指引」，本校學生入住宿舍時，請遵守以下事項：

一、入住時請先於各宿舍體溫量測站自行量測體溫，並填寫「自我通報健康關懷表」(如附檔)，再至服務臺辦理入住手續。

二、如果您有發燒(額溫≧ 37.5℃、耳溫≧ 38℃)、呼吸道症狀、咳嗽、喉嚨痛、全身無力等類似嚴重特殊性傳染性肺炎症狀，請立即自行前往就醫，並與值班輔導人員聯絡(0910-631-831或0919-099-119)，請暫勿入住宿舍。

三、同學如從國外入境台灣，必須於檢疫場所完成14天居家檢疫且檢測通過後，於校外完成7天自主健康管理，身體無異狀始得入住學生宿舍。

四、請同學於舍區配戴口罩、勤洗手、保持社交距離等指引，以共同維護舍區健康與安全。

In response to COVID-19 situation, also in line with the policies of Central Epidemic Command Center regarding overseas students entry procedure, and guidelines of Ministry of Education concerning ‘University dormitory pandemic prevention measures’, we have set down the following regulations for living in the NCCU dormitory,

1. Before moving in the dorm, please take body temperature on your own and fill out the NCCU COVID-19 Disclosure Statement.

2. If you have symptoms such as high fever (forehead temperature≧ 37.5℃, ear temperature≧ 38 ℃), shortness of breath, difficulty in breathing, cough, sore throat, extremely fatigue, muscle ache, please go to the hospital for treatment and are prohibited from moving into the dorm.

3. If you enter Taiwan from overseas, you have to go through 14 days of ‘home quarantine’ at government designated place, after that you have to pass the related medical test as well as finish 7 days of off-campus Self-health Monitoring process. When the above procedures are completed and healthy, you are allowed to live in school’s dorm.

4. All students in dorm are advised to wear face mask, wash hands frequently and keep social distance so as to keep a healthy dorm environment.
