One Semester Exchange Student Health Exam Form C Notice

Check Item：

1. Measles and rubella immunization record ,which document is acceptable as long as the vaccines were given no earlier than 12 months of age or provide proof of positive measles and rubella antibody titers.

2. Chest X-ray for Tuberculosis (Within 14 days after arriving in Taiwan)

Time: Sep 4^th ,2020  (Fri) 14:00~16:00

Venue： Si Wei Hall 1^st floor

Important Notice:

1.Please bring your passport and passport copy to the medical examination

(1) If the immunization has been completed in your home country, you may bring the certificates of immunization to Taiwan(both Measles and rubella are needed, if you only have one, you still need to take the vaccine), and submit the COPY version of documents to registration on Sep 4th to avoid re-taking the vaccine. (The document is acceptable as long as the vaccines were given no earlier than 12 months of age).

If the immunization CAN NOT be provided or NOT in the form approved by NCCU, students should take the vaccine directly at the medical examination day.

(2) Student may choose to submit COPY for certificates of immunization(MMR) (or the positive titer laboratory result) and the chest X-ray examination report(Only the report within 3 month is accepted. Only the paper report is accepted, the X-Ray CD file is not acceptable.) to suffice the medical examination requirements. Please provide the English version in the form approved by NCCU only.

NCCU approved form : Only the ENGLISH VERSION with the HOSPITAL STAMP will be acceptabled by NCCU. Students,who provide other language version or without the hospital stamp,should take the vaccine and X-ray examination directly at the examination day.

2. Medical examination Fee : Pay cash at the scene(Check Item as following)

3.Please dress casually on the medical examination day.

4.After the examination, Form C will be given to the students and NCCU student health center will keep the copy for record.

5.If you can not attend the group Health Exam on Sep 4th, please done within 14 days after arriving in Taiwan.
