Student group insurance

To file NCCU student group insurance claim, you need to provide the following documents:

1. Application forms
2. Original copy of medical diagnosis certificate
3. Hospital receipts (original or certified true copy)
4. Certificate of tuition payment for the semester which the medical treatment occurred.
5. Photocopy of your bank passbook in Taiwan and ARC(the front and the back)
6. Fractures: Additional "Hospital X-ray Disc" is required.

■Notes:

• The insurance claim must be applied for within 2 years after the accident or illness occurred.
• If medical treatment occurred overseas, English version of local medical diagnosis certificates and hospital receipts are required.
