※Health examination information

※Conditions for health examination subsidy:

1. Public education employees over 40 years old in the school's establishment (excluding dependents)
2. The subsidy is once every two years (if you apply in 110 years, you will not be able to get the subsidy until 112 years)
3. The subsidy amount is NT$4,050.
4. You can take one day of public leave

※Application process for subsidy:

• When applying, please attach the original receipt and fill out the "Faculty and Staff Health Examination Application Form" to the third team of the Human Resources Office to apply for the subsidy (Please click here to open a new window to download from the Personnel Office website)
• This subsidy is not limited to the contracted hospitals of our school. Any receipt of health examination can apply for subsidy.
