1. Inquiry for information related to physical examination for new students

1. Academic Affairs Office → "Freshman Service Network" with relevant links at the bottom right of the homepage → Master's and Doctoral Programs/Bachelor's Classes → Items on the left - "Freshman Physical Examination"
2. Academic Affairs Office → Physical and Mental Health Center → "Service Business Items" - Health Care → Student Health Examination → New Bachelor's and Transfer Students/Master's and Doctoral Classes

2. How to download student health information card form

1. Academic Affairs Office → Physical and Mental Health Center → "Form Download" - "Medical Services" - "Student Health Information Card"
2. Academic Affairs Office → "Freshman Service Network" with relevant links on the lower right side of the home page → Master's and Doctoral Programs/Bachelor's Classes → Item on the left - "Freshman Physical Examination" → Instructions for the Health Examination of National Chengchi University's Master's and Doctoral Programs/Bachelor's Classes in the 114th Academic Year →114年8月18日(一)至8月26日(二)開放網站線上填妥"National Chengchi University Student Health Information Card"After confirming the front data, print it again

3. Date of on-campus physical examination

 February 1.114, 8 (6) Health Checkup for Master and Doctoral Programs

 2.114 / 8 / 31 (sun)Bachelor's degree class and transfer student health examination

8. If the on-campus physical examination cannot be conducted on that date and time, it can be conducted on August 30-31.On-site alternate.

5. If you miss it114年8月18日(一)至8月26日(二)For those who fill out the "National Chengchi University Student Health Information Card" online on the website, please download the blank "Student Health Information Card" by yourself during the physical examination, fill in the front information, and bring it to the physical examination unit for examination.

114. If you go to a public or private hospital for a medical examination, the medical examination report is valid for three months (starting from June 6, 1).
