2024 Health Examination for National Chengchi University Freshmen and Transfer Students

NCCU extends a warm welcome to all incoming first-year students! To help protect your health and legal rights, the University implements health examination in accordance with the “Implementation Rule for Health Examination for National Chengchi University Student”. The health examination should be completed Within one month of the start of the semester (by Oct. 8 2024). Students who fail to complete the health examination procedure within this time will be asked to leave University dormitory accommodation, and may be issued an official warning or minor demerit in accordance with Article 3 of the “Implementation Rule for Health Examination for National Chengchi University Students”. Please note that iNCCU account may not be accessible if the Health Examination is not completed within two weeks after the first day of school.

From August 19th to August 31st, please fill out the “National Chengchi University (NCCU) Student Health Information Card” online and choose the health examination method that is preferable to you).

The explanation below is divided into four sections: 

I.On-Campus examination

II.Examination at a University-designated institution

III.Examination at an approved public or private hospital

IV. Submit a health examination report for the current year (dated between July and September, 2024)

 

 

I. On-Campus examination   

1.Health examination time: Please arrive at the appropriate time and date for the examination.

3.Fee: NT 650, to be paid at check-in

4.Examination notice:

(1) From August 19th to August 31st, please fill out the information on the front side of the “NCCU Student Health Information Card” online (you do not have to print it out) by August 31st (Saturday).

(2) During the three days prior to the examination, please try to maintain a normal diet and normal sleeping habits; do not eat or drink excessively. On the day of the examination, you may eat breakfast as normal. Please wear comfortable shoes or sandals that can easily be taken off to facilitate the examination. If you are pregnant, you should notify the nursing personnel to not take a chest X-ray.

5. To ensure that the health examination proceeds smoothly, please arrive on your designated date. Graduate students who are unable to come on Sep.7th may attend the undergraduate session on Sep.8th. Undergraduate students who are unable to come on Sep. 8th may attend the graduate session on Sep. 7th.

6.Accessing health examination results: All reports are expected to be opened for online inquiry on meddle of October.

 

II. Examination at a University-designated institution: Chi Hsin Clinic

1.Please fill out the information on the front side of the “NCCU Student Health Information Card” online beforehand, print out the card (two pages) and bring it to the clinic.

Please contact the clinic in advance to book an examination.

2.Health examination time: August 26th (Monday) to September 23rd (Monday)

3.Fee: NT 650

5.The clinic offers health examinations during the following times:

      Monday thru Saturday: 13:00-17:00 (Check-in time until 16:30)

7.To avoid long waits, we recommend that you arrange to have the examination on a weekday.

    The health examination fee for students failing to complete the examination by the specified deadline (September 24th ) will be increased to NT 750.

 

III. Examination at an approved public or private hospital:

1.Please fill out the information on the front side of the “NCCU Student Health Information Card” online beforehand, print out the card (two pages), and bring it to an approved public or private hospital for the health examination. After getting the report stamped with the official seal of the hospital performing the examination, you should mail the report to the Health Service Section, Office of Student Affairs, ensuring that it arrives by September 23rd Monday. (Please keep in mind that it normally takes14 to 16 working days for a hospital to issue a health examination report. Please be sure to get your examination as early as possible!)

2.After mailing in the required materials to the Health Service Section, please call or log in to https://moltke.nccu.edu.tw/SSO/startApplication?name=stuhealth  to check whether it has been received.

 

IV. Submit a health examination report for the current year (dated between July and September, 2024)

*The examination items must conform exactly to the content of the National Chengchi University Student Health Information Card; any items that are not included will need to be examined separately

1.Please make a photocopy of the health examination report, and note your name, department, and telephone number on it.

2.Please fill out the information on the front side of the “NCCU Student Health Information Card” online beforehand, print out the card (two pages).

3.The above two items should be mailed (using registered mail) to the Health Service Section, Office of Student Affairs, by September 23rd.

4.After mailing in the required materials to the Health Service Section, please call or log in to  https://moltke.nccu.edu.tw/SSO/startApplication?name=stuhealth  to check whether it has been received.

 

 Note:

  1. Students taking an extended leave of absence, performing military service, or interrupting their studies, but retaining their registration as a student, are not required to take the health examination; the health examination can be postponed until recommencing their studies.
  2. Students whose families are registered with their local city, township, or rural township office as low-income households should bring documentary evidence of the above to the nursing station when attending the health examination in order to waive the health examination fee.
  3. According to Regulations for the Implementation of Health Examinations for Students and Regulations for Governing Visiting, Residency, and Permanent Residency of Aliens, I authorize NCCU to disclose the information on the Student Health Form to the NCCU Physical and Mental Health Center and the health examination contracted hospitals for student healthcare administration.
  4. Article 3 of the Implementation Rules for Health Examinations for National Chengchi University Students stipulates that students who will not be able to take the physical examination within the specified time limit should submit an application for deferral prior to the expiry of the time limit; a deferral may then be approved at the discretion of the University.
